Duties:Patient Care Coordinator

• Responsible for formulating the patient's plan of care with input from the Interdisciplinary Team (IDT) and for overseeing its implementation and for coordinating all hospice services to patient and families.

• Graduate of approved school of nursing that prepares for RN exam required.

• BSN from accredited school of nursing desired.

• Three years as an RN in clinical setting required with an additional three years healthcare nursing required.

• Minimum one year supervisory/ management experience required.
